Description

The aim of the workshop is to offer an interactive, case-based, “fireside” panel discussion looking at multidisciplinary team involvement in the care of the adolescent heart transplant patient.

Learning Objectives

LEARNING OBJECTIVES:
1. Using two unique post-heart transplant patient journeys, explore the role and involvement of the multidisciplinary team.
2. Highlight successful outcomes and challenges in adolescent heart transplant care.
3. Emphasize the critical role of multidisciplinary care in supporting the adolescent transplant patient throughout their journey.
